The "To Hell and Back" CH-46 medevac patch honors the Marine helicopter crews who flew the Boeing Vertol CH-46 Sea Knight into the most dangerous landing zones on the battlefield to evacuate wounded Marines. The CH-46 — affectionately known as the "Phrog" — served as the Marine Corps' primary medium-lift helicopter for over four decades, and its medevac missions in Vietnam, Iraq, and Afghanistan represent some of the most courageous flying in military aviation history. When Marines were hit, it was the Phrog crews who answered the call, flying into hostile fire to bring the wounded back alive.

Medevac and CASEVAC missions are among the most dangerous in military aviation. Phrog crews flew into hot landing zones where the enemy knew exactly where the helicopter had to go — to the wounded Marine on the ground. Pilots descended through enemy fire, crew chiefs and gunners suppressed threats from the ramp and windows, and corpsmen worked frantically to stabilize casualties during the flight back to the surgical team. The "To Hell and Back" motto captures the reality of those missions perfectly: the crews flew into hell to bring their brothers home, and they did it every single time the call came in.
