On the morning of June 6, 1918, 1st Battalion, 5th Marines attacked Hill 142 to prevent German flanking fire against advancing French forces — the opening blow of the Battle of Belleau Wood. This patch carries the Indianhead insignia as worn by the battalion that struck first and set the tone for the most celebrated battle in Marine Corps history.

1st Battalion, 5th Marines was the lead assault element on the morning of June 6, 1918, when the 4th Marine Brigade launched its attack into Belleau Wood. Tasked with seizing Hill 142, the battalion advanced across open terrain into German machine gun positions, fighting through grain fields and dense undergrowth. The battalion's attack was part of the larger 2nd Infantry Division effort that ultimately cleared the wood over twenty brutal days of combat. Throughout the remaining battles of WWI — Soissons, Blanc Mont, and the Meuse-Argonne Offensive — 1st Battalion, 5th Marines continued to serve at the tip of the spear.
